What's the exact platform specs to run this on a real machine, with sound? (I guess, the compomachine specs?)

Photon, you need a machine which has a graphics card capable of VESA Mode 0x105 (1024x768 resolution with 256 colors) and a soundcard which is capable of playing MIDI. The graphics are tested on real DOS (notebook booted MSDos and FreeDos). The sound plays fine on WinXP Dos (and DosBox of course), i don't have an actual "SoundBlaster" to test it on a real DOS. Judging from the DosBox config you would have to setup your soundcard like this :

i can confirm that sound works on AWE64+Win98, don't tried yet on real dos and I expect that AWEUTIL will suck ;) (no wavetable connector on soundcard, btw)
