A color scheme or something would have been cool. And I'd have liked it better without that little window. But that "two effects on the screen separated by a wavy line" thing was a nice idea.
Well, doing fullscreen stuff in Java isn't that easy, at least last year when Java 1.4 wasn't released yet. You can try out a c++-port of it which is basically the same, but more smooth and in fullscreen, at http://www.martin.st/caffeine/.
