• I found some stuff as early as 1979 from someone called "the whip" but they're just messages embedded inside the cracked programs.
  
  That's earliest instance of a message/intro put in front of a game i could find (they were called "screens" back then).

• You're right it's from 1982 but on the title screen it says "Copyright 1980 TSR HOBBIES, INC.", maybe it's an adaptation of a wargame or something.

• There was no c64 in Europe before 1983 (and it was released in the US at the end of 82).
  The file is the same that can be found in the HERO game, check the csdb link.
